HEALTH INFORMATION

Alert the school nurse regarding any new information about the following.  Information will remain confidential and will only be shared on an as needed basis with your child’s teacher(s):

  • Allergies (especially food allergies)

  • Injuries

  • Illness

  • Medications

  • Special accommodations

  • Other medical issues concerning your child

  

MEDICATION POLICY:

  • Medication can only be administered by the nurse following receipt of proper medical forms completed by both child’s medical provider and the parent.  Updated forms are required each school year

  • All medications must be in the original bottle or package with student’s name on it.  For nebulizer treatments, you must supply tubing and mask.

Children are NOT allowed to transport medication

Medication must be brought to the health office by an adult and picked up at the end of the school year

HEALTH SCREENINGS:

Health screening are mandated by the New Jersey Department of Education as listed below:

HEIGHT, WEIGHT, AND BLOOD PRESSURE:  Yearly for all grade levels
VISION SCREENING: Kindergarten, 2nd, 4th, 6th, 8th grades

HEARING SCREENING:  Kindergarten, 1st, 2nd, 3rd, 5th, and 7th grades

SCOLIOSIS SCREENING:  5TH and 7th grades

If your child has an abnormal screening, a referral will be sent home.  Please follow up with your health care provider as soon as possible.
